Policy Proposal/Impact Statement: Required as a prerequisite to policy development or revision

CSU Policy Template: Required after approval of Impact Statement by Executive Committee

Fast-Track Policy Template: For new policies or policy revisions that are limited in scope and impact to one department or division, do not have campus-wide impact, and can be approved without substantial stakeholder impact by the responsible Vice President, without Operations Committee or Cabinet approval. Contact OPC for guidance before choosing this form.

Existing Policy--Library Record Entry Request: Used to add an existing, approved CSU Policy to the Policy Library. Documents such as procedural manuals and guides and reference guides may also be added to the Library. Contact OPC if you have questions about submitting a document for inclusion in the Policy Library.
